To protect everyone involved, all sessions are audio recorded.
Please NOTE: I DO NOT provide hypnosis for people who have serious mental disorders or Substance Use Disorders (SUD - serious alcoholism or drug addictions). Sudden cessation of these disorders may affect the person adversely and cause them to suffer from withdrawal symptoms or could even be life threatening. These disorders need to be addressed by medical professionals first. If I unknowingly begin a session with someone who begins to show signs of SUD, I end the session. Also, a person is always able to end a session themselves, even while under hypnosis.
YOU AGREE AS THE CLIENT:
I have informed Griff Rain of any medical or psychological issues that may impact or be impacted by hypnosis.
Should I become aware of any adverse feelings or experiences related to this process, I agree to inform my hypnotist (Griff Rain) immediately
I understand I am in control and if I become uncomfortable, I can end the session at any time.
Harmful reactions to hypnosis are rare, but may include: dizziness, headache, nausea, drowsiness, anxiety or distress, and sleep problems. If any of these reactions occur after my session, I agree to contact Griff Rain as soon as possible.
I understand that the methods of hypnosis include relaxation, breath work, creative visualization, positive affirmation, mindfulness, self-awareness development, and other techniques which may produce physical and emotional responses, for example crying, which are normal and to be expected.
If any medical symptoms progress or become acute, I agree to seek medical attention from a licensed healthcare provider. In the event of a medical emergency or if I feel suicidal, I will call 911 or other emergency help.
I agree to continue medication as prescribed by my attending physicians and understand that hypnotherapy is not a substitute for medical care.
I understand hypnosis is neither a means for diagnosis nor treats any medical or mental health condition, instead it offers tools of self-discovery and awareness to compliment any medical treatment prescribed by a physician.
I affirm that hypnotherapy is appropriate for me and does not conflict with existing medical or psychiatric treatment and if I have any adverse reactions that I believe are a result of hypnosis, I agree to contact Griff or seek medical attention if needed.
I am over age 18, and consent to hypnosis services offered by Griffin Rain (If this is for someone under 18, a parent or guardian must accompany them.)
Michel Griffin Rain agrees to provide professional services in accordance with acquired training and experience to facilitate the client’s attainment of treatment goals.
I agree to engage in the process of hypnosis with Griffin Rain.
Waiver of Liability
By checking the box below, you, the client, voluntarily agrees to be the recipient of hypnotherapy sessions and accepts full responsibility for any and all effects arising from the hypnosis sessions.
You, the client, shall hold Michel Griffin Rain harmless for any physical or mental effects of hypnosis.
Audio Recording
Client agrees that the hypnotherapy sessions are recorded for our mutual protection.
The Hypnotherapist does not share recordings.
Warranty
No warranty is given, expressed or implied, for satisfactory results from the hypnosis session(s).
